All BTI surgical instruments and prosthetic components are supplied clean and hermetically sealed, but not sterilized (except in the case of implants). Therefore, before the first use, sterilization is recommended. The recommended sterilization process is described below.

Prior to packaging at the factory, all instrument's joints, hinges and threads are lubricated. Prior to use, it is necessary to clean and then carefully rinse the instruments to avoid problems such as stains, deposits on the joints, etc.

After using the instruments, proceed with cleaning and sterilization according to the procedures described as follows:

CLEANING AND STERILIZATION PROCEDURE

Decontamination:

1.- Cleaning and rinsing.

2.- Cold sterilization (sterilization).

1.- MANUAL CLEANING

For manual cleaning of the surgical instruments, BTI recommends:

- Using protective gloves.

1. In the operatory where the instruments are used, they are placed on a tray and covered with a towel dampened with room temperature distilled water to prevent blood and body fluids from drying prior to cleaning. Then, they are taken to the processing area.

2. Remove debris with the brush with bristles which are not too hard and wet with room temperature tap water.

3. Disassemble the instruments (if applicable).

4. Submerge the instruments in a detergent (disinfectant or non-disinfectant) which can dissolve most of the debris, including that deposited on critical parts such as joints. Use a neutral detergent in accordance with the concentration and adequate temperature recommended by the manufacturer. Rinse with room temperature tap water to remove any detergent.

5. Place the instruments in an ultrasonic cleaner in a solution with a neutral pH detergent in the correct concentration and temperature following the manufacturer’s recommendations. Rinse with distilled water at room temperature. The solution should be changed as soon as turbidity or suspended particles are detected since debris inhibits the cleaning ability of the equipment. Cleaning in the ultrasonic is not recommended for any instruments with cutting parts or for instruments with tungsten carbide components.

6. Visual inspection.

7. Lubricate (if applicable) with a water soluble lubricating solution. Do not use silicone based lubricants or oil because they inhibit subsequent sterilization processes.

8. Dry the instruments carefully by hand. Use a non-shedding cloth. Be sure that the instruments are completely dry.

9. Assemble the instruments (if applicable).

During the cleaning process never mix instruments of different materials (chrome, aluminum, stainless steel).

2.- STEAM STERILIZATION

Sterilization (pre-vacuum).

Instruments should be placed in thermoseal autoclave bags.

- Temperature: 132ºC–135ºC (270°F-275°F); Exposure time: 3 – 4 min.

Autoclave sterilization - instruments with clamps and joints should not be closed (metal to metal) since it can cause breakage due to heat, moreover, closure prevents water evaporation at the contact points. The autoclave cycle should be followed to completion without interrupting the drying cycle in order to avoid risk of instrument oxidation.

Selection of autoclave packaging (heat sealable bags) for steam sterilization taking into consideration the following:

- Appropriate material for the method and cycle.

- Strength of the packaging.

- Type of autoclave packaging.

- Integrity of packaging required for the instrument design.

Bags should be introduced into the autoclave with the paper face up to encourage water evaporation, keeping them in the same position when removed from the sterilizer to avoid oxidation risk of the contents.

It is important to be certain not to sterilize any oxidized instruments or materials together with the rest of the instrumentation, since it increases oxidation risk during joint sterilization.
